INTRODUCTION TO COORDINATING CARE FOR PATIENTS

The first collection of “Discovering Value-Based Health Care” (modules 1-3) focused on defining value and understanding the concepts of outcomes and cost measurement. In this second collection, “Value-Based Health Care Delivery” (modules 4-5), we will discuss how to design care systems to better deliver value-based health care to patients and our communities

Delivering value-based health care means we need to fundamentally refocus care delivery to be designed around the needs of individual patients and their medical conditions. This module will explore models for coordinating care for patients.
